One registration form must be filled out for each participant for each program (soccer and baseball).

Circle the desired selection for the designated program.

Regular registration fees apply for all registrations received after March 15, 2010. Select the appropriate fee based on the date SAA will receive the registration.

Write the registration fee on the “Fee” line in the Amount section.If desired and an Intertown Soccer Program U12, U14 or U18 selection has been chosen.

Add the “Fee” and “Select” entries in the Amount section together, write the summary on “Subtotal” line.

If two other 2010 SAA participant registrations have already been paid in full from the same family, then write $ 10.00 on the “Discount” line in the Amount section.

Subtract the “Discount” line from the “Subtotal” line in the Amount Section, write the resultant amount on the “Total Fee” line, reflecting the fee required for this participant. Fees for more than one participant may be combined on one check.

Complete the participant information section. Include your e-mail address if you would like to SAA to contact you on-line. Please circle at least one area where you could provide volunteer help.

Read and sign the approval section. Sorry, unsigned registrations can’t be accepted. Make out your check for the amount identified above on the “Total Fee“ line to “Scottsville Athletic Association Inc”. Mail the completed registration form and check to:
